Yes everything included with lifetime warranty on the "growl" never returning.
Mine only sounded like a bag of nails for a few seconds on a hot start but it did make me really cringe when it did it making me reluctant to drive it hence only 700 miles in last 12 months. It was the original vanos from new though so had done well in that respect having clocked up 87k in total.

On a hot start, I'll check that I thought it was a cold start, I was aware it affected the performance, I would imagine it was a slow deterioration on the vanos, so the reduction in performance was not that obvious I'm assuming
Yes performance seemed to have tailed off over a long period but it has come back with a bang now
Cold starts was where it was relatively quite. It is hard to tell from the video but the growl was always there just very loud on occasions.
